What is pre-visit planning?
Pre-visit planning happens when a clinician or care team reviews patient notes, lab results, medications changes, and other relevant information before seeing a patient. This helps patients and clinicians make the most of each visit.
Why is it important?
Pre-visit planning saves time during appointments, identifies gaps in care, improves care coordination, and increases patient engagement.
What we are doing:
We are testing and refining pre-visit planning tools and processes: templates, checklists, care gap reports, and an infrastructure to make this information readily accessible to clinicians and care teams. We are also strengthening patient
involvement using questionnaires, patient-reported outcomes, and shared decision-making measures to help patients play a more active role in their care.
